IPH Health Center is required to serve all patients regardless of insurance status or ability to 
pay for services. However, you may be expected to pay a nominal fee for services, based on sliding fee scale federal poverty guidelines and eligibility determination. Proof of income is required. IPH staff will provide assistance with insurance enrollment for those patients who are eligible. This requirement is fundamental to the IPH Health Center model as the majority 
of the Health Center’s patients are low-income, underinsured or uninsured persons with no 
other primary health care.

Immunizations to students ages 10-21 Monday through Fridays.
Hepatitis B immunizations are FREE.  Parent or Guardian MUST accompany student and immunization record should be brought to appointment. Appointments are preferred; walk-

Currently offering immunizations for Hepatitis A & B, measles, mumps, rubella, chicken pox, 
and shingles for ADULTS. By appointment ONLY. Walk-in clinics are on a first-come / first-serve basis till they reach their capacity. Child MUST be accompanied by a parent or legal guardian. Visit Information:
Please bring any previous immunization records with you to your visit. A parent or legal
guardian must be present for the entire visit for a child to receive her or his immunizations.
Immunization Fees:
We will not decline services to clients if they are unable to pay. Washtenaw County Health Department accepts Medicaid and Blue Cross Blue Shield of MI. Please have your card 
when scheduling & bring it with you to your appointment. Currently, we cannot bill other 
private insurance. We accept cash, checks or Visa and Master Card. If using cash, please 
bring bills smaller than $50. An administration fee ($15 for children and $20 for adults) is 
charged per injection.
&
Some insurance providers may reimburse you for some of the costs. We will give you a 
receipt, but it is your responsibility to contact your insurance company to determine cov-
erage and request reimbursement. If you are not sure if your insurance covers immuni-
zations, call the toll-free number on the back of your insurance card and ask.
